You will need the following documents to complete enrollment:

• Child Birth Certificate (child must by 4 by September 1)
• Proof of Residency (current utility bill, signed lease, government letters (TANF, SSI, Foodshare, W2)
• Proof of Income (W2 statements, Federal Income Tax Return)
• Immunization Records

Yes. Our parents are required to pay a $40 monthly co-payment fee per family.

No. Our center is closed during most observed holidays and during severe, inclement weather. Keep in mind that if Milwaukee Public Schools is closed, our center will be closed. You can always check with your local news channels for a regular posting of businesses closed.
